Case Manager
The Tanzania Women Research Foundation (TAWREF), a Tanzanian NGO registered in 2010, aims to enhance health care services, education, shelter, and economic well-being for marginalized groups. We urgently seek a competent and experienced Tanzanian to join our Epidemic Control Project.
Position Description
The Case Manager will collaborate with Community Engagement Officers and Biomedical Officers to implement an HIV prevention program targeting at-risk populations in the region. Responsibilities include:
- Liaising with Districts Community-Based HIV coordinator (DCBHCO).
- Enrolling clients from the community through collaboration with facilities and Peer Navigators.
- Managing client appointments and ART refills, tracking missed appointments, and supporting community-based ART services.
- Mentoring Peer Navigators, acting as a liaison between them and Facilities, and supervising peer navigation teams.
- Tracking and reporting completed and non-completed referrals, following up on open cases, and ensuring all beneficiaries receive outcomes.
- Conducting site visits for at least 70% of the time, working with Data Manager/M&E Officer to report data, and preparing reports related to tracking services.
- Participating in meetings at CSO level and CHMTs, documenting best practices for linkage, and performing other duties as assigned by line manager.
Qualifications and Experience
- Bachelor’s degree or Diploma in Clinical Management, Nursing, Social Work, Counselling, or Psychology.
- At least 3 years of experience in Case Management for community-based HIV services.
- Prior experience working with literate and non-literate audiences; proficiency in participatory methods.
- Capacity to handle data with high integrity. Experience on KP programme is an added advantage.
- Experience in supervising a field team.
- Good interpersonal skills, communication skills in Kiswahili and English, ability to work in a team.
- Computer skills (e.g., Microsoft Excel, Word).
Other Information
Contract Duration: 3 months with possibilities of extension. A minimum of two referees from individuals familiar with your personal integrity is required. The Applicant should be ready to start work immediately.
